Repeat Prescriptions

Please request your repeat prescription using the slip on the right hand side of your last repeat prescription only, and either fax or post it to us enclosing a SAE if it needs to be posted back. If you do not have the slip, please put your request in writing with your name, D.O.B, address, telephone number, and details of the medication you require.

Your requests should be dropped in the box provided clearly marked ‘Repeat Prescription’ located in the reception area.

The number of hours from requesting a prescription to availability for collection by the patient is 48 hours or less (excluding weekends and bank/local holidays).

We accept telephone requests only from our registered housebound patients.
